Provider Description

Happy Healthy Minds program (otherwise known as Family Mental Health Support Service) supports young people in the Snowy Monaro Region (Cooma, Jindabyne and Bombala and the smaller communities within Snowy Monaro).
The program works with children/ young people 0-18 years old and their families who are at risk of or affected by mental illness. We provide short term intervention (up to 3 months) and intensive (up to 12 months) early intervention support for children/young people and their families which may include assessment and identification of needs, practical assistance and home-based support and making interagency referrals as appropriate. Our case management involves a targeted support plan being developed in consultation with families and supporting families to achieve positive outcomes in collaboration with different stakeholders.
Happy Healthy Minds provides community outreach, mental health education and community development activities.
The program has a small amount of brokerage that is allocated to each client to support them practically to achieve and support case goals identified by young person in their individualised support plan; application for funds are assessed by program manager of the Happy Healthy Minds program.

Criteria For Accessing Services

The Happy Healthy Minds Program targets young people aged 0-18 and their families/carers who are showing early signs of, or are at risk of developing a mental illlness.
