Provider Description

Age Matters supports vulnerable and isolated older people who need extra help to access services and supports. This includes seniors who are homeless or are at risk of becoming homeless. Age Matters provides case management services for eligible older people under the care finders program, working alongside seniors to access appropriate housing and linkage to community services to support ageing in place.
How to Access Services

Referrals can be made via the form on our website (agematters.org.au), by email (info@agematters.org.au) or by telephone call to 13 44 78, where you can ask to make a referral to the Age Matters program.

Criteria For Accessing Services

Support is available to seniors who are:
Isolated with no trusted support person
Needing intensive support to access services
Homeless or at risk of becoming homeless
Frail and aged 65+ or prematurely ageing and aged 50+ if homeless
Aboriginal and Torres Strait Islander aged 50+
