Provider Description

The Age Matters Assistance with Care and Housing (ACH) Program offers support to low-income seniors whose living situation is not secure. This includes seniors who are homeless or are at risk of becoming homeless. Age Matters provides case management services with the goal of working alongside seniors to locate sustainable and appropriate housing and connecting to appropriate community services to support ageing in place.

How to Access Services

Referrals can be made via the form on our website (agematters.org.au), by email (info@agematters.org.au) or by telephone call to 13 44 78, where you can ask to make a referral to the Age Matters homelessness program.

Criteria For Accessing Services

Support is available to seniors who are:
• Living on low incomes
• Homeless or at risk of becoming
homeless
• Frail and aged 65+ or prematurely
ageing and aged 50+
• Aboriginal and Torres Strait
Islander aged 45+
• Living in insecure or inappropriate housing
