ABOUT THE PROGRAM

This project is delivered in partnership with Illawarra Shoalhaven Local Health District.

The Homelessness Mental Health Program (HMHP) Project assists people who have a mental health condition and are currently homeless or are at risk of becoming homeless. HMHP Coordinators work collaboratively with the assigned case manager(s) to improve outcomes for clients regarding their accommodation status and management of their mental health condition, and/or provide items through brokerage funds available to assist people to establish or maintain a tenancy.

REFERRAL CRITERIA

  • Over 18 years of age
  • Homeless or at risk of homelessness
  • Diagnosed with a mental health condition
